New Patient Registration

All newly registered patients will be asked to complete a questionnaire and have a consultation with the practice nurse within one month of registering. Medical treatment is available from the date of registration.

Please note that a new patient check must be carried out failure to attend your new patient check may result in you being removed from the practice list

Pre-Registration with the practice

PreRegistration If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

A ppointments
The practice operates a comprehensive, urgent on the day appointments, telephone apppointments and pre-bookable appointments that are available up to Seven days in advance. We also ensure that patients who are unable to get an appointment on the day to see a GP the GP will call you this is referred to as a Triage system

Cancelling your Appointment
If you are unable to attend an appointment with one of the doctors or nurses, please telephone or use the link at the bottom of this page to cancel your appointment.

Out of Hours
If you require an emergencycall outside the surgery opening times, please telephone either Deerness Park or Bunny Hill Primary Care Centre, where you will be given a pre-recorded message on how to contact the doctor on call.
